AFSCME employees, please use the PTO Sellback form for 2008 to sell back a portion of your accrued paid time off. The form is to be signed and submitted **DURING** the benefits open enrollment period of October 13, 2008 through October 24, 2008.
Please make sure that the signature and "DATE SUBMITTED" date happens either on or between these dates. Any form received with a date prior to or after will not be processed and returned. Please make sure the form is read and completed carefully.
Once the form is complete, it is to be forwarded to the Plant Payroll and Accounts Payable Office for processing. Accrual banks will be re-verified and adjusted accordingly in January (This is different than prior years, when the time would be taken out of the accruals as the forms were received). The sell-back will be paid in the last biweekly pay scheduled for January 16, 2009.
NOTE: **PLEASE DO NOT SEND FORMS TO THE CENTRAL PAYROLL OFFICE, THIS CAUSES THEM GREAT CONFUSION**
